Getting To Know You & Your Oral Health

New patient forms are a way for us to gain insight to your oral health and any previous dental work you have had. It’s also an opportunity for you to make note of any services or information you have an interest in. Upon checking in to your first appointment, we will request that you complete these forms so the team can get a patient file started for you. We can also collect insurance information and keep it on file for future use.

Our office is in-network with Delta Dental. We accept all out-of network plans and will bill accordingly. If you have any questions about insurance, please get in touch with our front desk team for help! We do not accept medicaid at this time.
